• 2017-2018 Physical Form is good for one calendar year
• Emergency Medial form- is needed for each sport
• Registering Your Athlete this must be completed and a current physical must be turned in or on file in the Athletic office before trying out or attending the first practice.

ELIGIBILITY FOR FALL ATHLETICS
To be eligible for FALL ATHLETICS, current 7th grade students  must have passed a minimum of 5 subjects that receive a grade or a pass/fail in the 4th quarter grading period and have a 1.5 GPA.
