Parochial Transportation Request for 26/27 School Year
If your student will be attending one of the Parochial schools listed below and they will require Lebanon to transport them, please complete the transportation request in the following link: 26/27 Parochial Transportation Request
Please complete this request by 3/28/26 in order to recieve busing at the beginning of the 26/27 School Year.
Parochial Schools that Lebanon provides busing for are as follows:
- Lebanon Christian Elementary
- Lebanon Christian Intermediate
- Lebanon Christian Secondary
- John 23rd
- Middletown Christian
- St Francis
- Summit Academy Elementary
- Summit Academy Secondary
- Marshall
- Lebanon United Methodist (LUMP)
Transportation Request/Payment in-lieu-of Form
By completing this form, you are requesting transportation and accepting or rejecting payment in-lieu-of transportation for the 2025/2026 school year. This form must be completed by 9/1/25 in order to receive payment in-lieu-of for the 2025 -2026 school year. Once the school year ends, verification of enrollment and residency will occur and then payment will be mailed in the month of July 2026. Requests made after 9/1/25 may result in a pro-rated payment based on the date of the request.

You MUST complete this request form every year to receive payment In-Lieu-of. After verification of enrollment and residency at the end of the school year, payment will be mailed in July of 2026. If the request is made after 9/1/25, the payment may be pro-rated.
